Patient Transfer

 

As an Evercare member it is helpful if you can provide your previous patient records to your Evercare GP so they can access your medical history. This is a common practice amongst GP clinics. Below we’ve outlined some simple steps to make this happen.

1. Please contact your previous medical clinic and request a transfer of your records.
2. Ask them to email your records to Evercare in  XML format. Your records can be emailed to our clinical team at clayfield@evercare.com.au.

If your previous practice requires your request in writing, you can use this handy template (PDF format)  to provide to your previous clinic. This will need to be filled in and emailed or sent to them.
